  Auto-select Speaker in Call During audio calls, automatically switch to the speaker when not holding your iPhone to your ear.

  Hi, you can find this setting in iOS settings> Accessibility > voiceover > audio, and there is a toggle called auto select speaker in call. You want to switch that on.
