The easiest way would be to open a word document, and just answer the questions on there. Save the document, then send it to the person through email.

Seems like it is easy with Google forms, did a YouTube search for Google forms using NVDA and there are a few walk throughs. I haven’t needed to create a form in a long time but I do know survey monkey is quite accessible as well but I would try Google first.

I have had a little bit of experience with creating a Google form. Last year, the BITS affiliate with ACB did a whole class on the Google workspace, and Google forms was part of the material they covered. It really wasn't that bad to create a form. The presentations were recorded. If you are a member, you can go onto their website and give the material a listen.
