What about a dentist nurse? She knows that I'm blind.

in order for it to be certified. You need to have either a nurse signature, a Doctor signature, because they have to know that you are blind. That’s the way to get it certified. I did paper application, back in 2016. At the visiting nurse that I had she signed off on it. It has to be some sort of healthcare professional.
